5 as Agency) is a contracting agency of the Public Employees'

7 WHEREAS, the Public Employees' Retirement i_aw requires that
it a contracting agency determine whether an employee of such
agency in employment in wnich he 15 classified as a focal safety
10 member is disabled for purpeses of the Public Employees ' Retire-
11 ment Law and whether such disability is " in(Iustrial" within
12 the meaning of such law;
13 WHEREAS, an application for disability retirement of
I4 Raenette Eldridge employed by the Agency in the position of
15 Police ❑fficer has been filed with the Public Employees '
16 Retirement System; and
11 WHEREAS, the C17Y OF SAN JACINTf) has reviewed the
18 medical and other evidence relevant to such alleged

11) disability,
20 1 . NOW, THEREFORE BE IT RESOLVED:

21 a. Raenette Eldridge' s physician, George S.
22 Watkin, ^1,D. , declared her on September 12,
2XI 1988 to oe permanent and stationary.
24 b. Employer 's physician, V. Prabhu bhalla, M.D. ,
25 declared her on Octaber '.7, 1988 to be
26: oermanent and stationary.

27j C. On Novemher 20, 1988, Raenette Eldridge's
28 employer offered her vocational rehabilitation .
29 d. On December 15, 1988, Raenette Eldridge declined
N) tier emDlnver ' s offer for renabilitation.

Fh at the CITY OF SAN JACINTq finds and determines
9 and it does hereby find and determine that Raenette
3 Eldridge is incapacitated within the meaning of the
4 public Employee' s Retirement Law "or per£ormance of
her duties in the position of Police Officer .

1s to thr Worker 's Compensation Appeals Board for a
14 determination pursuant to Section 21026 whether such
15 disability is industrial ; and

17 3. BE IT FURTHER RESOLVED that the member was separated
18� from her employment in the position cf Police Officer

1J � effective February 2, 1999 and that no dispute as to

the expiration of such leave rights under section
21 210W.4 of the Government Code is pending.
